Student and Parent Handbook Acknowledgement Form

It is the responsibility of parents and students to read the Middlebury Monterey Language Academy Student and Parent Handbook prior to participating in our 2012 Summer program. The information contained in this Handbook outlines the Policies, Rules and Regulations that govern our program. The signature on this Acknowledgment Form by each student and their parent is required in order to participate.

Our signatures indicate that we have received, read and understand the Middlebury Monterey Language Academy Student and Parent Handbook. Our signatures also indicate that we agree to all the policies, procedures, and rules as outlined in this handbook and that if we have any questions regarding the interpretation or application of the policies, procedures and rules in this handbook we should direct those questions to our Regional Site Manager.
